Counselor Request Form

Schedule Change Request FormThis form must be submitted to School Counseling in order to change any classes by the deadlines for the 2026-27 school year.

Feel free to download the form here or get one from the School Counseling office at the beginning of each semester.

Parent Override Course Recommendation: This form should be submitted if your child was not placed in an Honors, Pre-AP, AP, or Dual Enrolled course due to teacher recommendation, but they still want to switch into that level course.

This form can be downloaded or picked up in the School Counseling Office.
